Person-Centered Planning: A Vision for the Future
About this session:
Person Centered Planning puts the individual at the center of the decision making process as a vision for the future is created. It focuses on discovering the person’s gifts, skills and capacities and on listening to what really matters to the person. It is used as a life planning model to enable individuals with disabilities to increase their personal self-determination and improve their own independence.
The goals of person-centered planning are to:
• Gain a deeper understanding of participants interests and support needs
• Assist the focus person in gaining control over their own life
• Develop meaningful goals that match participant interests
• Design and implement meaningful support plans for participant growth
This discussion group will cover the Person-Centered Planning process from the participant, family and provider perspective, share success stories and discuss ways to overcome challenges in the planning process.
Danielle Azzalino is the Director of Adult Services at Gatepath, located in San Mateo County. She manages site-based Learning Resource Centers, a Learning Employment Campus, Employment and Community Access programs that serve adults with special needs and developmental disabilities.
Elizabeth Zirbes is a Supervisor for Gatepath's Community Access program. This program engages each participant's unique goals through skill building in the areas of Health and Fitness, Independent Living Skills, Personal Advocacy, Pre-Vocational Training, and Travel-Training. By creating tailored programming in a low-ratio, integrated environment, we are able to offer our participants experiences that help prepare them for the lives they want to lead. Community Access is one of Gatepath's Adult Services programs that is redefining what Person Centered Planning means. We believe that organizations of the future need to look beyond documentation and toward meaningful People-First Service.
